Mechanistic and biologically based models in epidemiology; a powerful underutilized tool

Abstract
Mechanistic and biologically based mathematical models of chronic and behavioral disease processes aim to capture the main mechanistic or biological features of the disease development, and to connect these with epidemiological outcomes. These approaches have a long history in epidemiological research and are complementary to traditional epidemiological or statistical approaches to investigate the role of risk factor exposures on disease risk.In the article by Simonetto et al. (Am J Epidemiol. XXXX;XXX(XX):XXXX–XXXX)), the authors present a mechanistic, process-oriented, model to investigate the role of smoking, hypertension and dyslipidemia on the development of atherosclerotic lesions and their progression to myocardial infarction (MI). Their approach builds on and brings to cardiovascular disease the ideas and perspectives of earlier mechanistic and biologically based models for the epidemiology of cancer and other chronic diseases, providin g important insights into the mechanisms and epidemiology of smoking related MI.We argue that although mechanistic modeling approaches have demonstrated their value and place in epidemiology, they are highly underutilized. We call for efforts to grow mechanistic and biologically based modeling research, expertise and awareness in epidemiology, including the development of training and collaboration opportunities to attract more students and researchers from STEM areas into the epidemiology field.

An Independent Seizure-Onset Zone in Medial Temporal Lobe Found by 18F-FDG PET Imaging Besides Epileptogenic Periventricular Nodular Heterotopia

imageA 23-year-old man with drug-resistant epilepsy was admitted for presurgical evaluation. The epileptogenic zone could not be derived from seizure semiology and scalp electroencephalographic monitoring definitely. MRI showed periventricular nodular heterotopia in occipital horn of left lateral ventricle with high FDG uptake on interictal 18F-FDG PET scan, whereas the hypometabolic zone in the left medial temporal lobe was also found on PET with no abnormality on MRI. Stereoelectroencephalographic implantation was performed to identify the seizure-onset zone. Two independent epileptogenic foci located in periventricular nodula r heterotopia and left hippocampus were validated by stereoelectroencephalographic monitoring and the outcome of subsequent thermocoagulation.

FDG PET/CT in Eosinophilic Solid and Cystic Renal Cell Carcinoma With Lung Metastases

imageEosinophilic solid and cystic renal cell carcinoma is a recently described emerging renal tumor. Usefulness of FDG PET/CT in the management of eosinophilic solid and cystic renal cell carcinoma has been rarely reported. We describe FDG PET/CT findings in a case of eosinophilic solid and cystic renal cell carcinoma with lung metastases. The primary renal tumor and its metastases showed intense FDG uptake with SUVmax of 29.3 for the primary tumor and 21 for the lung metastases.

Prostate Cancer With Solitary Bone Metastasis in the Mandible

imageA 79-year-old man with prostate cancer found to have a solitary bone lesion in the mandible on initial staging bone scan. The sclerotic lesion had a sunburst appearance on CT. As biopsy was nondiagnostic, the patient underwent segmental mandibulectomy. Postoperative pathology results confirmed as metastasis from prostate cancer.

We report a case of epithelioid hemangioendothelioma with pleura and bone metastases on 68Ga-FAPI-04 PET/CT in a 65-year-old woman who underwent surgery and chemotherapy.
