Latest Insights into the Pathophysiology of Migraine: the ATP-Sensitive Potassium Channels

Abstract

Purpose of Review

Migraine remains a challenging condition to treat, thus highlighting the need for a better understanding of its molecular mechanisms. This review intends to unravel a new emerging target in migraine pathophysiology, the adenosine 5′-triphosphate-sensitive K+ (KATP) channel.

Recent Findings

KATP channel is a common denominator in the cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) mediated intracellular cascades, both of which are involved in migraine. Intravenous infusion of KATP channel opener, levcromakalim, provoked migraine attack associated with dilation of extracerebral arteries in all persons with migraine.

Summary

Preclinical and clinical studies implicate KATP channels in migraine initiation. KATP channel is a novel therapeutic target for the acute and preventive treatment of migraine. Future studies are warranted to provide a better understanding of the role of KATP channel subgroups in migraine.

Clinical Evaluation a New Treatment for Infection After Ventriculoperitoneal Shunt

To explore a new surgical treatment for infection and obstruction of ventriculoperitoneal shunt in hydrocephalus. Two cases of post-operative infection of ventriculoperitoneal shunt were analyzed retrospectively. One case was cryptococcal infection, the other case was Acinetobacter lwoffii. The number of cerebrospinal fluid cells was high, the infection of ventriculoperitoneal shunt was generally complicated with abdominal obstruction, and the hydrocephalus was aggravated again, The authors try to pull out the drainage tube at the end of abdominal cavity for external drainage, combined with intravenous antibiotics, completely control of infection, and then use the original shunt device for intraventricular jugular shunt. The authors explore that this method is simple, safe a nd effective, and it is an effective and feasible method for the treatment of infection after ventriculoperitoneal shunt. Chiasmatic Ridge: Incidence, Classification, and Clinical Implications

Objective: The study aimed to identify the chiasmatic ridge (CR) morphology and to discuss its surgical significance. Methods: The suprasellar region of 90 adult dry skulls with unknown ages, sexes, or nationalities was examined to record the shape and incidence of the CR. Results: The CR was found in 14 (15.56%) of 90 dry skulls. The ridge was classified into 3 types, taking into account its shape, incidence (presence/absence), and the risk of residual tumor formation. The absence of the CR was named as Type 1 (76 cases, 84.44%). The bony extension without a hidden area was identified as Type 2 (8 cases, 8.89%), while the bony extension with a hidden area as Type 3 (6 cases, 6.67%). Conclusion: Type 3 CR (which has an evident hidden area beneath the ridge) should be taken into account by neurosurgeons to eliminate possible residual tumor risk during removal of tumors (eg, meningioma) with subfrontal or supraorbital approaches. Endoscopic Endonasal Management of Pterygopalatine Fossa Tumors

Surgical removal of pterygopalatine fossa (PPF) tumors with endoscopic endonasal approach is still challenging. The present study aimed to evaluate our endoscopic endonasal management of PPF tumors based on the tumor pathology and purpose of the surgery. This comprised both a single nostril approach for biopsy and a binostril approach for complete resection of benign and noninfiltrating tumors. Based on this strategy, 12 patients underwent endoscopic endonasal surgery for PPF tumors between 2013 and 2018. The patients' data were analyzed retrospectively to demonstrate the significance of our treatment scheme. The surgery was terminated only after taking a biopsy specimen in 6 patients. Other 6 patients underwent gross total resection or bulk tumor reduction. Final patholo gical diagnosis was malignant in 6 cases and benign in the remaining 6. Post-operative treatment was needed in 7 patients. Four operations for the 6 patients who underwent either debulking or radical surgery were performed by the binostril approach; while 5 surgeries for the 6 biopsy patients were performed by the single nostril approach. Postoperative complications were tolerable. Endoscopic resection should be adopted preferentially for benign tumors that can be removed in a piecemeal fashion. However, as most malignant tumors were impossible to resect with a negative margin, priority should be given to tumor biopsy using an endoscopic approach, which is less invasive than an open approach, and an appropriate treatment customized to the pathological diagnosis should be administered.
